Rodents, roaches, flies and serious sanitation lapses dominated this week’s list of Miami-Dade, Broward and Palm Beach restaurants that failed Florida health inspections, according to reports from the Florida Department of Business & Professional Regulation.
The inspections — both routine and complaint-based — led to closures or stop-sale orders until restaurants corrected violations and passed re-inspections, often the following day.

Inspectors documented a disturbing scene when a live rodent entered through the back door, collapsed on the floor and died. The manager then disposed of the rodent and returned to the dishwasher area without washing his hands.
Inspectors also found live roaches near the dishwasher, utensils stored in dirty water, improperly sanitized dishwashers, and a bar handwashing sink missing soap, paper towels and hot water.

Complaint inspection | 12 violations, two high priority
Inspectors found rodent droppings around cooking equipment and more than a dozen dead roaches near a fire pit and food-holding areas. Flies hovered and landed on prep tables and cutting boards. Officials also noted heavily soiled walls, ceilings and vents throughout the kitchen.
Ichimi Ramen Bar
2330 Salzedo St., Coral Gables
Routine inspection | 30 violations, nine high priority
Inspectors found extensive evidence of roach activity, including significant droppings. The restaurant failed two inspections before finally passing on the third attempt.
Las Vegas Cuban Cuisine
7015 Broward Blvd., Plantation
Routine inspection | 3 violations, two high priority
Wastewater backed up through floor drains in the kitchen, forcing employees to walk through contaminated water. Staff could not isolate the affected area.
Little Caesars
1436 Lantana Rd., Lantana
Complaint inspection | 7 violations, four high priority
Inspectors ordered a stop-sale after finding pizza dough prepared from open, gnawed-on bags of dry dough mix. Rodents left more than 40 droppings inside and around boxes of deep-dish dough sheets and prep tables.
Officials also cited a broken paper towel dispenser at a handwashing sink.
Miyi La Cubanita Cuban Cafe
1553 E. Sample Rd., Pompano Beach
Routine inspection | 13 violations, three high priority
Inspectors found black mold-like buildup inside the ice machine and multiple dead roaches on clean dishes and behind cooking equipment. Live roaches hid in equipment wheels, and staff handled clean utensils after cracking raw eggs without washing hands.
Nana’s Food For You
312 N. Federal Hwy., Hallandale Beach
Routine inspection | 7 violations, two high priority
Live roaches crawled inside cleaning buckets and soap dispensers. Inspectors also found a dead roach inside the employee handwashing sink, which lacked hot water, soap and paper towels.
Old Tom’s Sports Bar Restaurant
9525 North Kendall Dr., Kendall
Routine inspection | 23 violations, seven high priority
Inspectors reported massive fly activity — more than 90 flies — landing on food and prep areas. They issued multiple stop-sale orders for temperature-abused foods, including raw chicken, cheese, shrimp and vegetables.
They also cited unsafe glove use, dirty slicer blades, standing water throughout the kitchen and a complete lack of soap at handwashing sinks.
Oyanko Sushi
6655 Boynton Beach Blvd., Palm Beach County
Routine inspection | 9 violations, two high priority
Rodents left droppings on top of the dishwasher, and inspectors found uncovered food in the freezer. Dead flies hung from sticky traps near the dishwashing area.
Pack Supermarket & Cafeteria
8235 NE Second Ave., Miami
Routine inspection | 22 violations, four high priority
Inspectors counted more than 50 rodent droppings throughout the kitchen, including on counters, floors and near food storage. Follow-up inspections still found rodent activity and live roaches inside a walk-in cooler.
Punta Cana Restaurant
9119 Taft St., Pembroke Pines
Routine inspection | 16 violations, three high priority
Inspectors found live and dead roaches throughout the kitchen, standing water under sinks, dirty cooler shelves and uncovered food stored near handwashing sinks.
RC Caribbean Restaurant
3760 NE Third Ave., Deerfield Beach
Routine inspection | 10 violations, three high priority
Heavy grease buildup covered the floors and prep areas. Inspectors also found hundreds of rodent droppings on prep shelves and about 20 live roaches under unused equipment.
